Transvaginal MeshVaginal mesh, transvaginal mesh, pelvic mesh all refer to basically the same line of  products, urogynecologic surgical mesh.  The use of surgical mesh came about in the 1950s for use in reinforcing and repairing internal tissues.  It wasn’t until the 1990s that this mesh was being used for vaginal surgery to treat pelvic organ prolapse (POP), more commonly prolapsed bladder.  The widespread use of surgical mesh prompted medical device makers to view this area as very profitable niche, and in 2002 the first vaginal mesh kits were approved by the FDA.

Mesh Complications

It has been just over a decade since the approval of the first vaginal mesh kits.  Since that time an alarming number of adverse events have been reported by the FDA.  Numerous mesh complications have been documented by women who have underwent vaginal mesh repairs for various reasons, mainly pelvic organ prolapse and stress urinary incontinence or SUI.  The most common mesh complications are urinary difficulties, infection, pain, severe pain, organ perforation, exposed mesh fragments,  pain during sexual intercourse, vaginal scarring, reoccurring prolapse, bleeding and serious emotional difficulties.  In some cases the mesh actually eroded!

Prolapsed Bladder Surgery

Prolapsed bladder surgery is performed in severe cases of pelvic organ prolapse.  This commonly occurs when the vaginal walls are weakened and the upper organs drop down.  This puts pressure on the vagina making it push forward causing discomfort. Many times extreme discomfort.  Pelvic organ prolapse is caused by the weakening of the abdominal muscles or vaginal walls.  This causes the prolapsed bladder too.  There are two basic strategies for surgery in this area.  One is the traditional method of using stitching and the other is the now controversial approach utilizing vagina mesh kits.  Both procedures have risks involved with them.

Vaginal PessaryUsing a vaginal pessary is an increasingly popular alternative to prolapse surgery.  A vaginal pessary is a temporary device used up to 3 to 6 months.  It is intended to support your internal organs.  It is installed high in the upper regions of the vagina.  When installed correctly most women will not notice it is there.  In fact, many women report being able to go about their daily routines with out any discomfort, even sexual intercourse.

In less sever cases of vaginal prolapse, certain targeted exercises can be performed.  Mainly exercises focused on strengthening your pelvic floor muscles or the kegel exercises.  Be sure to wear supportive underwear.  There are also forms of biofeedback therapy to help retrain your muscles in the affected areas.

Because estrogen has been proven to make skin thicker and increase the size of the pelvic floor tissues hormone replacement therapy can be used.  Oral estrogen can come with side effects, so topical estrogen replacement creams are normally recommended.
